Meet Sarah,  age 42.

This is one year of her cancer journey.5

 

1. Diagnosis

Diagnosed with breast cancer after a positive mammogram, ultra-sound, and biopsy. Referred to oncology surgeon and oncologists.

2. Identification

Cigna Healthcare’s predictive analytics identified Sarah and outreaches by call or letter.

3. Engage

Sarah is assigned a Personal Oncology Nurse Advocate who helps her through her entire care journey. Today she finds Sarah in-network providers and advises on resources.

4. Behavioral Support

Oncology Nurse connects Sarah to oncology-specific behavioral support to manage anxiety with her new and unexpected diagnosis.

5. Care

Sarah meets with her oncology surgeon who is in-network and orders an MRI. MRI approval via text.

6. Care

Sarah meets with her medical team and is presented a treatment plan.

7. Expert Opinions

Oncology Nurse contacts Sarah and her provider for approval for an outside review by expert oncologists.*

8. Treatment

Expert opinion recommends an alternative treatment accepted by Sarah’s oncologists.*

9. Ongoing Support

Regular check-ins with her Oncology Nurse. Supports with benefits questions, clinical trials, nutrition guidance, side effects, claim issues support, etc.

10. Care

Sarah receives call notifying her the infusion site is out of network. She is provided with alternative locations and Sarah decides which works best.*

11. Care

Sarah completes her chemo regime. She is supported with regular outreach by her Oncology Nurse through surgery & radiation.

12. Care

Sarah receives her oncology maintenance medications via mail with 24/7 pharmacy support. Sarah enrolls in a manufacturing assistance program and has $0 co-pay.*

13. Behavioral Support 

Sarah continues to work with an oncology-specific behavioral specialist to manage anxiety about the cancer returning.
